The gap this program closes

Your accountant tells you what happened last month. That’s about it. Is your fee schedule priced right? Which payer quietly pays you 20% less than the others? How much revenue never made it through your billing process last year? Most practice leaders can’t answer those questions, and it was never their fault. Medical school skips this material, and no practice manager’s job description came with a training program. You learn it on the fly, or not at all.

That gap has a cost. In the practices PMI analyzes, five common fixes (fee schedule pricing, screening capture, after-hours codes, zero-pay recovery, denial reduction) often add up to six figures a year. Money already earned. Just never collected.

About your instructor

Paulie Vanchiere founded the Pediatric Management Institute and has spent more than twenty years analyzing the financials of independent pediatric practices across the country. He grew up inside the business of pediatrics — his father built and led a large private pediatric practice — and he knows the other side of the exam room as the parent of a child with complex medical needs. He is the author of the upcoming textbook Pediatric Practice Management: The Fundamentals. Over 14 sessions you work the same analysis method PMI uses in client engagements, applied to your own practice data — whether you own the practice, manage it, or are about to buy into it.

THE 14 SESSIONS

1
OCT 13, 2026Foundations & Revenue Intelligence

Set up the data conventions that make every later analysis accurate, then dig into revenue per encounter and trending. You’ll leave knowing what your practice earns per visit and where revenue hides.

2
OCT 27, 2026Billing Performance & Revenue Pipeline

Measure how much of what you earn actually gets collected, and how fast. Covers collection rates, A/R aging by provider, denials by root cause, and the true cost of your billing operation.

3
NOV 10, 2026Payer Analysis, Transparency & Negotiation

Rank your payers by what they actually pay, catch down-coding and underpayment, and pull the CMS transparency data showing what peers get paid. Then build the data package for your next negotiation.

◇  Thanksgiving break — no session Nov 24
4
DEC 8, 2026Pricing & the Business of Vaccines

Price your fee schedule using the same RBRVS system payers use, and find the codes priced below what payers would pay. Then work the real economics of your vaccine program, from margin to spoilage.

◇  Holiday break — no session Dec 22
5
JAN 5, 2027Cash Flow, Budgeting & Financial Statements

Learn why a profitable practice can still miss payroll, and how to read all four financial statements. Build an annual budget and a 12-month cash flow projection you’ll actually use.

6
JAN 19, 2027Provider Productivity & Scheduling

Measure provider work objectively with wRVUs and benchmark against national percentiles. Then optimize the schedule: seasonal templates, utilization, and the revenue cost of no-shows.

7
FEB 2, 2027Overhead, Expenses & Cost Management

Calculate your true non-vaccine overhead rate and benchmark every expense category. Know your cost per encounter, the floor below which any payer contract loses money.

8
FEB 16, 2027Compensation, Contracts & Compliance

Compare the five compensation models and what behavior each rewards, then run the affordability model on your own numbers. Covers bonus design, APP margins, and Stark/AKS compliance.

9
MAR 2, 2027Patient Panel, Retention & Growth

Analyze your panel’s size, age shape, and retention, and find the age bands where well visits go missing. Includes the HEDIS immunization measures that drive quality payments.

10
MAR 16, 2027Front Desk, Operations & Process Improvement

Treat the front desk as the financial department it is: time-of-service collection, verification, and financial clearance. Then map workflows and assign clear ownership with PMI’s Responsibility Matrix.

11
MAR 30, 2027Practice Value, Structure & Succession

Understand what your practice is worth and what drives that number up or down. Covers corporate structures, balance sheet health, financing, and the succession timeline.

12
APR 13, 2027Partnership Governance & Agreement Review

Score your partnership agreement against the 12-dimension governance scorecard and find the provisions most agreements miss. Capital calls, forced buyouts, disability, tail coverage, and profit allocation.

13
APR 27, 2027Customer Service, Satisfaction & Culture

Measure satisfaction across patients, providers, and staff, and connect the results to retention and revenue. Then assess the culture that drives every decision your practice makes.

14
MAY 11, 2027Leadership & Your 90-Day Action Plan (7–9 PM) CAPSTONE

Consolidate every metric from the program into your Practice Financial Scorecard and identify your three biggest gaps. You’ll leave with a 90-day action plan and present it for group feedback.
